Thank you for your interest in the Communities in Schools program. Please select who is completing the Parent Consent form today below.

In order to complete the form for themselves, a student must be 18+ years old on the day the form is signed. If you are not 18+ years old, a parent must complete the parent/guardian version of this form.

Please enter the student's birthdate.
We currently only accept electronic Parent Consent forms from a parent/guardian of a participant or a participant who is 18+ years old and can legally sign the form. Please speak with your Site Coordinator for additional options.

If you have any trouble using the signature field, you can utilize the checkbox to have the form sign for you based on your typed name.

Forms submitted with an incomplete name on either line will not be accepted.
